Payment and Refunds

Payment for services is made up-front and all transactions are final.

Charges

Services will be charged at the following rates:

• $50 – Telehealth Appointments (all types)
  • Initial and follow-up exams
  • Discussion and planning surrounding imaging results
  • Discussion and planning surrounding complex lab results (greater than 3 diagnoses)
• No charge:
  • Phone discussions regarding simple lab results (less than 3 diagnoses)
  • Letters to other providers
  • Referrals
